This Notice of Privacy Practices (“Notice”) is effective as of October 10, 2025.

This Notice describes how medical information about you may be used and disclosed and how you can access this information. Please review it carefully. Sparks Medical Corp, doing business as Weight Unlocked (“Weight Unlocked,” “we,” “us,” or “our”), is required by law to maintain the privacy of your Protected Health Information (“PHI”), to provide you with this Notice of our legal duties and privacy practices, and to notify you in the event of a breach of your unsecured PHI.
How We May Use and Disclose Your Health Information
We may use or disclose your PHI for the following purposes without your written authorization:
• Treatment: To provide, coordinate, or manage your medical care and related services.
• Payment: To collect payment for the services we provide, such as billing you directly for cash-pay telehealth services.
• Healthcare Operations: For quality assessment, staff training, accreditation, and administrative purposes to improve our services.
• Appointment Reminders and Communications: To contact you regarding upcoming appointments or follow-up care.
• As Required by Law: When required by federal, state, or local law, such as reporting abuse, neglect, or certain diseases.
• Public Health and Safety: To prevent or control disease, injury, or disability, or to report adverse events.
• Health Oversight Activities: To comply with audits, investigations, or licensing requirements.
• Judicial and Administrative Proceedings: In response to a court order or lawful subpoena.
• Law Enforcement: To comply with laws for reporting certain types of wounds or physical injuries.
• Research: In limited circumstances, for research approved by an Institutional Review Board.
• Business Associates: To individuals or entities that perform functions on our behalf who have agreed to maintain the confidentiality of your PHI.
• Coroners, Medical Examiners, and Funeral Directors: As necessary for their duties.
• Workers’ Compensation: To comply with workers’ compensation or similar programs.
• Serious Threats to Health or Safety: To prevent or lessen a serious threat to your health, safety, or that of the public.
Uses and Disclosures Requiring Your Written Authorization
Certain uses and disclosures of your PHI require your written authorization, including:
• Marketing communications not related to your treatment
• Sale of your PHI to third parties
• Most uses or disclosures of psychotherapy notes (if applicable)
If you provide authorization, you may revoke it in writing at any time, except to the extent we have already taken action in reliance on it.
Your Rights Regarding Your Health Information
You have the following rights regarding your PHI:
• Right to Access: You may request copies of your health records. Reasonable fees may apply for copies or mailing.
• Right to Amend: You may request that we correct or amend your health information if you believe it is inaccurate or incomplete.
• Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.
• Right to Request Restrictions: You may ask us to restrict certain uses or disclosures of your PHI. While we are not required to agree, we will honor reasonable requests.
• Right to Request Confidential Communications: You may request that we communicate with you through specific means or at a specific location.
• Right to Receive a Paper Copy: You may request a paper copy of this Notice even if you received it electronically.
• Right to Be Notified of a Breach: You will be notified if there is a breach involving your unsecured PHI.
Our Responsibilities
Weight Unlocked is required by law to maintain the privacy and security of your PHI, provide you with this Notice, and abide by its terms. We reserve the right to change our privacy practices at any time and to make new provisions effective for all PHI we maintain. If our practices change, we will update this Notice and post it at our offices and on our website.
Telehealth and Cash-Pay Services
Weight Unlocked provides telemedicine services using secure technology. These services are cash-pay only and are not billed to or reimbursed by insurance. All telehealth interactions are documented and protected as part of your medical record.
